  1. Visit the Switch to T-Mobile page at reference link 1.
  2. Enter your ported T-Mobile phone number into the section provided. A ported phone number means that you want to keep your phone number from your old carrier to your new carrier.
  3. Enter the last four digits of your Social Security Number or Business Tax ID into the section provided.
  4. Tick on I’m not a robot.
  5. Click on NEXT.
  6. Follow the rest of the instructions as prompted to complete your request for reimbursement. You may be asked to provide other personal and billing information. And depending on the reimbursement that you are requesting for, you may be asked to trade-in your device from your old carrier. Also, you will be asked to submit or upload your final bill from your old carrier.

How to Track your Reimbursement Request when you Switch to T-Mobile

  2. Click on CHECK STATUS under ‘Already Submitted?’.
  3. There are three ways to track the status of your reimbursement request: through your contact information, through the combination of your phone number and zip code, or through a tracking number.
  4. Track your reimbursement request through your contact information: enter your first and last names, street address and zip code into the sections provided. Click on CHECK STATUS.
  5. Track your reimbursement request through the combination of your phone number and zip code: enter your phone number and zip or postal code into the sections provided. Click on CHECK STATUS.
  6. Track your reimbursement request through a tracking number: enter your tracking number into the section provided. Click on CHECK STATUS.
